Consultations and Treatment Notes

Clinical teams should record visit details clearly so the next person reviewing the case can understand what happened without guesswork.

Before documenting

  • confirm you have opened the correct patient profile
  • review the most recent visit history
  • check whether there are existing reminders or follow-up instructions

During the consultation

Record the information your clinic requires, such as:

  • reason for visit
  • examination findings
  • treatment or medication given
  • advice shared with the owner
  • recommended follow-up

After the consultation

  1. Review the notes for clarity.
  2. Confirm any treatment items that should flow to billing.
  3. Add follow-up or revisit instructions if needed.
  4. Save the record before moving to the next patient.

Documentation tips

  • use consistent wording for common visit outcomes
  • keep notes specific and clinically useful
  • avoid leaving draft information incomplete at shift change
